• This program is for physicians on our hospital’s medical staff who choose to participate.
• There is no charge to you or the physicians who participate in this program.
• If you do not have a preferred physician for follow-up care, we can help you select one based on your needs, location, insurance and availability.
• Some physicians who are employed by an affiliate of our hospital, or participate in a clinical network with our hospital, may receive preference in the rotation.
• All physicians are independent practitioners and are solely responsible for the medical services provided to you.
